The Care Partner plays a vital role in coordinating and managing consumer care packages, ensuring that all services are delivered in a coordinated, person-centred, and cost-effective manner. This role supports consumers in maintaining their independence by assisting them to identify and access the services they need.

The Care Partner will provide:

  • Care Partner services to consumers eligible for Support at Home services funded by the Commonwealth Government.
  • Care coordination for consumers eligible for the HACC PYP Linkages Program.
  • Support Coordination for participants eligible for the National Disability Insurance Scheme (NDIS).

The ideal candidate will have strong communication and coordination skills, a commitment to person-centred care, and the ability to navigate complex service systems to achieve the best outcomes for consumers.

Your New Division

The Community and Public Health Services Division has a primary focus on improving the health outcomes of the communities in our region.  With a range of local and regional programs supporting place-based health promotion, prevention, care and support, they work collaboratively with other Divisions and regional partnerships and networks to support quality outcomes.  The Division has a strong leadership role in the Loddon Mallee Region and is committed to ensuring processes are in place to deliver on our Division.  Community Care Services conducts a high-quality case management service across the Loddon Mallee region. Case management supports people who are frail aged and younger people with a disability and their carers to live independently in the community.
