Salary per hour (exc. super): $52.70 - $64.83
Hours per fortnight: Up to 80 (including an ADO every 4 weeks if working 80 hours)
Employment Status: Fixed Term Full Time or Part Time (20/07/2026 - 18/07/2027)

The Discharge Navigator (Registered Nurse or Allied Health Professional) plays a vital role in improving patient flow and supporting timely, safe hospital discharges across acute medical wards at Bendigo Health. This role will operate over 7 days to include weekends and provide support across all acute medical wards
The Discharge Navigator role assists with access and flow for daily capacity management by early identification and prioritisation of patients with a planned discharge in the next 48 hours. This position reports into the Access and Demand Manager and works collaboratively with the Digital Command Centre, ward staff, treating teams, and internal and external providers to facilitate timely discharge.
The purpose of the Discharge Navigator role is to “act now” to assist with progression of care for patients through Bendigo Health wards, services, and the Emergency Department. The focus of this role is to ensure that there is an agreed EDD and discharge destination for all patients up to 48hrs prior to discharge, and that any delays to care progression are being identified and actioned.
This will allow discharges to be confirmed the day prior to discharge, with the aim to have two confirmed discharges on each ward the day before. The person in this role expedites discharges by taking direct action on specific tasks that are required or supports and facilitates the treating team to do so. The person in this role is expected to prioritise their work to enable patients to be discharged hours or days earlier in their stay. The Discharge Navigators will have a ‘home base’ Medical team on weekdays but will actively support other wards and services when needed, as directed by the Patient Flow Coordinator or Access and Demand Manager.
The Discharge Navigator and Patient Flow Coordinator roles will contribute to an improvement in the Bendigo Health access and flow KPI’s, pre-10am and 12pm discharges, Medical Admitted Length of Stay and Discharge Lounge utilisation.
